*treatment plan formation includes establishing therapeutic goals, recommended frequency/type of follow up, and action plan.

25 minute brief follow up or interim visit with more focused attention on active problems areas. Fee $200

50 minute follow up visit with focus on core issues and advancement of goals. Fee $375

50 minute In-home session or at venue selected by client. Available after first in office visit. Fee includes minimum travel time of 25 minutes each way subject to increase as a function of commute duration. Average fee: $750
